Farmland Application Documents

The following is offered as guidance to help you fill out and sign your annual Farmland Assessment package. We will supply your management plan and the activity maps, so don't worry about them.  Click the icons below for exact instructions on how to fill out your respective forms. Application for Farmland Assessment Form (FA-1)

Information contained in this form should be read for accuracy, signed and dated, and returned to Pine Creek Forestry. Two copies are sent to the tax assessor, one copy to the New Jersey Forest Service, and copies are kept as a record on file at Pine Creek. You should keep a copy too!

Supplemental Farmland Assessment Gross Sales Form (FA-1 GS)

The Supplemental Gross Sales Form should be checked for accuracy, signed and dated, and returned to Pine Creek Forestry. A copy is sent to the tax assessor and a copy is kept as a record on file at Pine Creek. A copy will be returned to you for your personal records.

Woodland Data Form (WD-1)                    

Woodland Data Forms should be read for accuracy, signed and dated, and returned to Pine Creek. A copy is sent to the tax assessor, the New Jersey Forest Service, and copies are kept as a record on file at Pine Creek. You should keep a copy. This form must be accompanied with a current forest stand map!


All Farmland Assessment documents and associated materials must be filed by August 1, of every year -No exceptions.

What you need:

1. Application for Farmland Assessment (FA-1)

2. Supplemental Farmland Assessment Gross Sales Form (FA-1 GS)

3. Wood Receipt(s) (If required by the Tax Assessor)

4. Woodland Data Form (WD-1)

5. 2020 Forest Activity Map (Supplied by Pine Creek)

*If you are participating in the Forest Stewardship program, a Gross Sales Form (FA-1 GS) is not applicable.
